IMPORTANT INFORMATION

- Please use one registration form per person. Photocopy if additional forms are required.

- If you are registering as a group, all forms for each participant must be mailed in the same envelope.

- All payments must be made in Australian dollars.

- Please be familiar with our cancellation policies.

- You will receive the tax invoice after payment of the 7 week Core Camp® 4 KIDS Program attended and all payments must be made in FULL before your child can attend their selected date and location
